We are currently looking to recruit • Full Time Home Care Manager Position with VISA/Sponsorships to permanent resident available

Home Care Manager
The Home Care Manager to coordinate the delivery of services to eligible HCP consumers in their homes.

Key Responsibilities:
• Home Care Manager for consumers ranging from Level 1-4 HCP • Assessing, coordinating, and reviewing client care needs within their budget • Assess needs of the consumer and develop strategies with consumer, family and health care professionals in accordance with all funding guidelines • Develop and manage individualised budgets reflecting My Aged Care support plans • Develop and implement individualised plans for consumers according to My Aged Care requirements to reflect all consumer goals and needs from ACAT assessments • Conducting annual reviews and constant liaising with consumers and family on day-to-day basis • Liaising with GP’s and allied health professionals • Conduct comprehensive assessments with field staff for consumer needs • High attention to detail. Exceptional time management skills and prioritise all day-to-day requirements and duties • Detailed weekly and monthly reports submitted in a timely matter reviewed with management to provide continuous improvement for all consumers and staff

Essential Requirements: • Diploma in Community Services • Experience working in Aged Care / Disability / Home Care (highly desirable) • First Aid / CPR / Manual Handling • Drivers Licence and own registered vehicle with comprehensive car insurance (required) • Current National Police Check (or willing to undergo one) • NDIS Worker Clearance Check • Covid-19 Infection Control Training • Full COVID-19 Vaccination
